When travelling to some foreign countries you may be required to have some Travel Vaccines before you go. The Surgery offers appointments with our Practice Nurses every day (Mon-Fri) for this.
In order for the Nurses to assess what you're likely to need against what you've already had you will need to complete our Travel Form (Please ensure you complete and return this form at least 8 weeks before you plan to travel).
You can come into surgery to collect this form from The Reception Desk or alternatively you can complete the form online by clicking HERE.
Please allow a few days for the Practice Nurse to review your form - Then contact the Surgery to arrange an appointment - If you have not already been contacted by the Surgery.
Please Note: There is a National/Global shortage of Hepatitis A and Hepatitis B vaccinations which has affected the Surgery supply. The Practice Nurse will discuss this with you at your appointment. However you may wish to contact private travel clinics to check their availability and attend there, Thank you.
