Due to the current COVID-19 Pandemic the practice has changed the way we manage face to face reviews, including annual reviews of long term chronic conditions. The Practice believes it is important that we continue to monitor your condition(s) and provide support until we can safely reinstate our usual face to face annual review appointments.
The Blundellsands Surgery has put a re-call system into place, to ensure you are invited for one review per year for your conditions. Our aim is to invite you to attend in your Birthday Month to act as a reminder for you too.

You will be re-called if you have one or more of the following conditions:

Please Note these reviews are in addition to any hospital clinics you attend for your conditions.
1. Your first invite will be in your birthday month via letter, telephone or text - you will be contacted on three occasions throughout the year if you do not respond to the previous invites.
2. You will arrange a Blood Test appointment with our Health Care Assistants (If you are signed up to Patient Online Services - you can book this appointment yourself, online).
3. A Joint HCA & Practice Nurse appointment will be arranged with you, for when your blood results are available.
4. You will have your review BP, Height, Weight and Lifestyle will be reviewed - If during your review the nurse feels you need to be seen more frequently than annually you will be advised of this during your appointment i.e. those with Diabetes.
Your annual review is an extremely important part of your care and therefore it is imperative we have all of your details correct in order for us to contact you.
