Please Note these reviews are in addition to any hospital clinics you attend for your conditions.
1. Your first invite will be via letter, telephone or text - you will be contacted a further time should you not respond to your first invite.
2. You will be asked to arrange a Blood Test appointment at the community clinics by using the following link: https://www.merseycare.nhs.uk/bloodtestsouthsefton or by calling 0151 285 4548.
3. A Practice Nurse appointment will be arranged with you, for when your blood results are available.
4. You will have your review BP, Height, Weight and Lifestyle will be reviewed - If during your review the nurse feels you need to be seen more frequently than annually you will be advised of this during your appointment i.e. those with Diabetes.
