Chronic Disease Reviews


The Blundellsands Surgery has put a re-call system into place, to ensure you are invited for one review per year for your conditions. Our aim is to invite you to attend in your Birthday Month to act as a reminder for you too.

 If you have any concerns about your review being overdue please contact the surgery.  


You will be re-called if you have one or more of the following conditions:

Atrial Fibrillation (AF)           

Coronary Heart Disease (CHD)

Chronic Kidney Disease (CKD)

Chronic Obstructive Pulmonary Disease (COPD)

Diabetes Mellitus


Heart Failure

Mental Health

Peripheral Artery Disease (PAD)

Rheumatoid Arthritis




Please Note these reviews are in addition to any hospital clinics you attend for your conditions.

1. Your first invite will be in your birthday month via letter, telephone or text - you will be contacted on three occasions throughout the year if you do not respond to the previous invites.

2. You will arrange a Blood Test appointment with our Health Care Assistants 

3. A Practice Nurse appointment will be arranged with you, for when your blood results are available. 

4. You will have your review BP, Height, Weight and Lifestyle will be reviewed - If during your review the nurse feels you need to be seen more frequently than annually you will be advised of this during your appointment i.e. those with Diabetes.

Your annual review is an extremely important part of your care and therefore it is imperative we have all of your details correct in order for us to contact you.
