Annual re-call for Chronic Diseases
Our practice is looking to update our system to ensure you are invited for one review per year for your conditions. These include diabetes, heart disease, stroke, high blood pressure, COPD, asthma, chronic kidney disease, peripheral arterial disease and rheumatoid arthritis.
Our aim is to invite you to attend in your birthday month to act as a reminder for you too. Moving to this system may result in you being invited sooner or later than you expected. If you have any concerns about your review being overdue please contact the surgery.
If during your review the nurse feels you need to be seen more frequently than annually you will be advised of this during your appointment i.e. those with Diabetes.
